Inserting all the boundaries into the set once didn't help. You're right that I can reduce the conditions and still trigger the problem. I've got it down to once cell, and applying no normal flux conditions on boundary 0 and 2, which gives me the error. Applying to only boundaries 0 and 1, which don't have an intersection, does not trigger the problem.
I haven't investigated it yet but for the sake of avoiding duplicate work, I've checked the test (with one cell) in as tests/deal.II/no_flux_10. It obviously fails right now.
