Please do not introduce eyesight, numeracy or general knowledge tests.
A literacy test might have some grounds (journalists), but in general,
captchas are worse than useless.

What benefit would anyone get from attacking the web form anyway?
Just avoid accidentals (email confirmation is fine, and probably a
good idea anyway if you want to be able to email them later) and all
should be well.
