Please do not introduce eyesight, numeracy or general knowledge tests. A literacy test might have some grounds (journalists), but in general, captchas are worse than useless. http://www.w3.org/TR/turingtest#security What benefit would anyone get from attacking the web form anyway? Just avoid accidentals (email confirmation is fine, and probably a good idea anyway if you want to be able to email them later) and all should be well.
