My ~/public_html/.htaccess file has:

Options +MultiViews

to make sure that MultiViews is enabled. But this yields the following
error:

  Internal Server Error

  The server encountered an internal error or misconfiguration and was
  unable to complete your request.

  Please contact the server administrator at webmaster@localhost to
  inform them of the time this error occurred, and the actions you
  performed just before this error.

  More information about this error may be available in the server error log.

/etc/apache2/mods-available/userdir.conf has:

[...]
  <Directory /home/*/public_html>
    AllowOverride FileInfo AuthConfig Limit Indexes
    Options MultiViews Indexes SymLinksIfOwnerMatch IncludesNoExec
[...]

i.e. with MultiViews already enabled. So, there's no reason to
issue an error.
