Please could you describe the setup where this happened?  Was /dev/sda
a removable USB stick?  Did you BIOS-boot from it?  What image was on
the stick?

Do you have a /var/log/syslog from this?  It would be even better to
uncomment the debug lines atop grub-installer before it runs, and get
/target/boot/grub/device.map.  (I'll do all this if you can answer the
above).
