You must have *very* unstable systems. :P If you have a
reasonably stable system you get a forced check every time you
reboot.

At any rate, e2fsck is a bit different from mdadm or zfs scrub in
that the former requires the fs to be unmounted, so it can't be
done in the background. So, monthly should be fine, and in the
cases where it's not there should be an admin who can change it.
