I bisected this and it it appears that c3045b is the one that introduced the problem. Looking closer it appears that I can not do ioctl(d->slave, TIOCSCTTY, 0) more than ones on freebsd without a new openpty() to get a new master pty (i.e. I can not use the slave fd again).
