hotplug did not enable my USB audio device automatically after a reboot. I
had to unplug and replug it to get the device enabled (the device was red in
'usbview').

I fixed it in my setup by moving /etc/rcS.d/S40hotplug to
/etc/rcS.d/S19hotplug, which is before module-init-tools, which causes the
usb modules to be loaded. Now the audio device is detected and enabled on 
reboot.

Perhaps hotplug should scan for un-enabled devices upon startup, and somehow it 
isn't?
