Making portrait pages on a4 paper (209.9028mm x 297.0389mm)
Then index.pdf is generated, but a line is displayed at the beginning of every
page showing:

Is this related in any way with the error in loading the DTD?
I also tried xmllint before processing with xmlto, but that doesn't
help.
