This package is built with support for MENUS and PANELS. FORMS support

The problem is probably the one explained in the INSTALL file:
/usr/lib/perl/5.8/CORE/form.h conflicts with /usr/include/form.h from
libncurses5-dev, and the Perl directory is first on the include path.
An easy way to fix this is to use libncursesw5-dev instead (see #462773);
then we have form.h in /usr/include/ncursesw and the directory first on
the include path.
Filing this to document the issue.
