The reason seems to be that gs puts setpagedevice commands at the beginning of each page, which is not redefined by ppdfilt, so discards the information that this page should only occupy a part of the page. (adding /setpagedevice {pop}bind 1 index where{dup wcheck{3 1 roll put} {pop def}ifelse}{def}ifelse which psnup adds there helps here, but as psnup is redinining some more commands (initclip, initgraphics, ...)
