I have sa_local_test_only=0 in /etc/amavis/amavisd.conf, yet it looks like
amavisd-new is not doing any networked test (RBL specifically).

In a mailbox with ~2000 marked spam messages, none has RCVD_IN_... in the
X-Spam-Status report; in another mailbox with >1300 false negatives there
are no RCVD_IN_... hits either. This looks suspicious to me; I doubt none
of 3300 spam messages would have passed through any RBL listed smtp
server.
Unfortunately there seems to be very little means of debugging amavis'
spam checking so I can't be 100% certain.
