It would be nice if readahead-fedora would schedule a profile boot
automatically when files in /etc/init.d/ change, to increase the
chance of an optimal boot.

I suspect adding a dpkg trigger would be a good way to implement this.

It already does that, thanks to a trigger that reacts on changes made 
to /etc/init.d and /lib/modules, and a monthly cronjob (which at some point I 
plant to get rid of so that it is purely based on real changes to the 
system).
