1.       Duplicate ES-IB. Balears should be ES-PM. Source:

2.       Duplicate LY-JA. Al Jabal al Gharbī should be LY-JG. Source: 
http://en.wikipedia.org/wiki/ISO_3166-2:LY

3.       Duplicate MA-TIZ. Tiznit is only a province and not a prefecture.

4.       Duplicate MA-ZAG. Zagora is only a province and not a prefecture.

Possible bugs relating to incorrect source data from ISO. Their website 
<http://www.iso.org/iso/country_codes/background_on_iso_3166/iso_3166-2.htm>  
states “Note that the characters after the separator are only unique within the 
subdivision list of one particular country. They can be (and have been) reused 
in the list of subdivision names of other countries e. g. ID-RI (Riau province 
of Indonesia) and NG-RI (Rivers province in Nigeria). So only a complete code 
element i. e. with the alpha-2 country code in front guarantees uniqueness.” 
I’m going to contact them about the following issues below.

 

1.       Duplicate CV-SL. São Lourenço dos Órgãos was recently added, so maybe 
the ISO made a mistake. Sources:

2.       Duplicate ID-MA. I don’t see any recent changes, but this seems to go
