is a <filename> or a <command> (and maybe other tags, too) inside a
<title>.
You can try it with the current version of the Debian Repository

I don't know whether it is really xmlto's error; AFAIK it might be a
problem with xmlto, xsltproc, or xmltex.

Not sure when, but the original issue seems to have been fixed.
The sample still fails because of an issue in docbook-xsl for which a
report has been submitted.
