There appears to be a faulty margin description in the FO's:

margin-left="1in - -4pc"

which results in inproperly set margins and the text "-4pc -4pc" appearing
at the top.

The issue was caused by a wrong logic setting the passivetex.extensins
parameter only if --extensions had been used. But the use.extensions
parameter is not related to passivetex.extensins or fop1.extensions.

Upstream fixed this in 0.0.24.
