   * What led up to the situation?

Upgrade to initramfs-tools 0.119 with /usr specified as /dev/vg0/usr in 
/etc/fstab.

   * What exactly did you do (or not do) that was effective (or
     ineffective)?

* downgrade to initramfs-tools 0.116 works.

* change /dev/vg0/usr to /dev/mapper/vg0-usr in /etc/fstab with 
update-initramfs -u works.

* boot before change in fstab works with vgchange -ay and ctrl +d to continue 
booting.

   * What outcome did you expect instead?

Should LVM-volume names not work as both /dev/<vg>/<volume> and 
/dev/mapper/<vg>/<volume> as before?
