opens the file in a*file*manager*  (nautilus). Which means it shows
the containing folder, not very useful.

I traced xdg-open, and it calls gvfs-open, which in turn uses
some mime applications.

Could you send me the trace? I.e.

sh -x /usr/bin/xdg-open foo.pdf

Actually the whole MIME settings is a big mess in Debian.

Do you think the situation is worse in Debian than in other distros? If so, why?

(But I do agree that the MIME situation in Linux is a mess in general.)
