The emacs backup file must have been the problem because I just have
this happen to me:

[EMAIL PROTECTED]:/etc/courier/smtpaccess# makesmtpaccess
Cannot store record for 127.0.0.1 - duplicate or out of disk space.
[EMAIL PROTECTED]:/etc/courier/smtpaccess# ls
default  default~
[EMAIL PROTECTED]:/etc/courier/smtpaccess# rm default~
[EMAIL PROTECTED]:/etc/courier/smtpaccess# makesmtpaccess
[EMAIL PROTECTED]:/etc/courier/smtpaccess#
