Package: lvm2 Version: 2.02.111-2 Severity: serious initramfs-tools 0.117 added mounting of /usr from the initramfs. Currently, if /usr is on a VG that is not also used by root or a hibernation device, that VG is not activated and the user must explicitly activate it through the emergency shell.
I believe it is necessary to add a new script stage to support this. I'll send a proposed script shortly.
