We got hit by this bug on Debian Jessie running on tracker.debian.org. The server would not accept new connections until we force a restart. A reload does not help. In our specific case, it appears to have started after some segfault in a child process: [Thu Aug 18 03:40:22.861570 2016] [core:notice] [pid 20708:tid 139737024104320] AH00052: child pid 31262 exit signal Segmentation fault (11) [Thu Aug 18 03:40:27.869195 2016] [mpm_worker:error] [pid 20708:tid 139737024104320] AH00288: scoreboard is full, not at MaxRequestWorkers [Thu Aug 18 03:40:27.869341 2016] [core:notice] [pid 20708:tid 139737024104320] AH00052: child pid 27873 exit signal Segmentation fault (11) [Thu Aug 18 03:40:27.869363 2016] [mpm_worker:warn] [pid 20708:tid 139737024104320] AH00291: long lost ch ild came home!
