Nope, it means what it says, though it sort of imagines that the reader is already familiar with the intricacies of OpenPGP certificates. "RSA and RSA" means "Make an signing- and certification-capable RSA primary key, and attach an encryption-capable RSA subkey to it". Similarly, "DSA and Elgamal" means "Make a signing- and certification-capable DSA primary key, and attach an encryption-capable Elgamal subkey to it".
