It looks like there are three problems here: 1) Driver for your floppy disk is either missing or not loaded. 2) This results in a situation that is not handled correctly by mountfloppy: none of the devices it looks for is present and still it tries to ask the "retriever/floppy/device" question. This results in the weird "Mounting 10 on" errors. 3) If no modules at all are included on the root floppy, the installer fails to generate a modules.dep file, even though the kernel _is_ modular. I'm cloning the bug for the second and third problem, but let's try to track down the first one which is the base cause of the problem. It looks very much as if the following udeb is just missing from the root floppy: floppy-modules-2.6.15-1-powerpc-miboot-di I've added that udeb. Let's hope that it does not make the size of the root floppy too big again. Could you please test again with images for 12-02 or later?
