executing /usr/share/apt-cacher/apt-cacher-cleanup.pl manually (or automatically by /etc/crond/apt-cacher) removes all deb-packages from /var/cache/apt-cacher, even if there is no newer versions in the debian repositories. According to the documentation and to the used behavior some timne ago, the newest packages should be left untouched.
