When I load a markdown document containing links that have the
ampersand (&) character in them, I get messages like this on the
terminal where I launched Formiko:

Entity did not end with a semicolon; most likely you used an ampersand
character without intending to start an entity — escape ampersand as
&amp;

It seems that Formiko doesn't properly escape the ampersands to HTML
(&amp;) when converting markdown to HTML.
