Hmm. This is oldstable. Why can't you run current stable? At least use the qemu-img utility from current stable if you can't upgrade whole system. Oldstable receives only critical fixes, and since this issue hasn't been reported before, during whole debian 9.0 stretch cycle.. You provided a relatively simple testcase, yet it needs quite some work still to find out where the bug has been fixed.
Converting with buster version (1:3.1+dfsg-8+deb10u3) is working.
