I was trying to create a custom cd of my installation
   * What exactly did you do (or not do) that was effective (or
     ineffective)?
I only follow the instructions from Debian Wiki page: 
https://wiki.debian.org/Simple-CDD/Howto

   * What was the outcome of this action?
