According to the manual page, you can set the X resource xpdf.geometry
to set the initial window size. Could you try this and see how it goes?

Yes, this allows you manipulate starting geometry of entire application but not 
afaics the window pane on the left side the provides the TOC indexing.

If there is a way to set the side pane, please let me know.
