The addresses are extracted before any rewriting takes place. "When Exim is receiving a locally-generated, non-SMTP message on its standard input...". So which options must I pass to exim, along with "-t", to get it to "receive a locally-generated non-SMTP message on its standard input"? Good luck finding the answer in the man page, or in the specification. The answer seems to be "none" but I don't know how I was supposed to figure that out, other than by trial and error.
It seems like a pretty typical use case to me, though. I think that something should be said near the top of the manual page about common use cases such as this. The most common options options can be put before the others, which would help a lot, but there should be some introductory text as well.
