I have a stock Debian (unstable) httpd.conf, with only one modification - no
/images alias.
mod_negotiation is loaded (that's the default, I guess).

Correct. Renaming resolved the problem.

I tried to get it to misbehave again, but I cannot. I've upgraded apache-perl
(and related) package(s) since, so I guess it is fair to assume that the fault
was on the apache side. I did check $r->uri at the time, and it was showing
correct (requested) uri. So what should I look at if I want to know if apache
played with the uri meanwhile?
