On each occasion the installer found the network PCMCIA card without problems (3Com Etherlink III 3C589 PCMCIA card). System appeared to configure correctly – downloading files over FTP. However, on re-boot the network card wasn't seen. Same problem on multiple occasions. Card services appeared not to boot. Cardmgr error 1775 - said there was "no pcmcia driver in /proc/devices" Re-installed Debian version 3.0 and card worked OK again. Multiple re-installs of version 4.0 failed as above.
