Perhaps the /etc/init.d/postgresql could be enhanced to actually read the pid from the .pid file and check if the process really exists? This is of course also not perfect (there could be another process running with this PID), but definitely an improvement over the current situation.
Alternative: put some code into /etc/rc.sysinit or so to just delete the .pid file unconditionally. If I'm not mistaken that happens automatically for all .pid files in /var/run.
