That sounds like some sort of interaction between Lesstif/Motif and the window manager. Once you exit Xpdf, the window manager is responsible for figuring out which window should have the focus. Maybe something in the Lesstif combo box implementation is confusing icewm? (I don't have icewm installed here, so I can't test this one.
