I've just released 0.540, which no longer dies when these arguments are
used together. I've added a note to the documentation about the fact
that a scalar for OUTPUT will always be a Unicode string, but would
welcome any suggestions for better language.

That's what I'd prefer to do but, for some functionality, specifically
US-ASCII, the XML serialiser needs to know about the encoding
(as some characters will need to be output as numeric references).

(This is also the case where using both arguments together makes sense.)
