you probably did not add samba.schema to your slapd.conf. The schema file is in /usr/share/doc/samba-doc/examples/LDAP/samba.schema.gz when you installed the "samba-doc" package. If you already included the file I will need the debug output of slapd. In /etc/ldap/slapd.conf turn on logging with the line "loglevel 256". OpenLDAP uses /var/log/syslog for log output.
