Yes, they indeed helped. Your spontaneous recreations of the "test"
database seem happen on every reinstallation or upgrade of the
mysql-sever(-4.1) package. The mysql scripts are responsible for this.
I already altered them not to create a user that can access this database
without password as it's the default for MySQL but I did not yet disable
the creation of the database itself.

So I fear you will have to live with "test" for a while now, but I leave
this bug report open as I might want to change it for the Debian package
somewhen.
