Yes, straightforward with mutt 1.5.18-1. But see below.

---------------------
1. Use a unicode locale, e.g. LC_CTYPE=de_AT.UTF-8
2.
echo 'set index_format="%4C %Z %{%b %d} %-15.15F (%4c) %?H?%H] ?%s"' >\
    /tmp/muttbug/muttrc
3. Launch mutt with minimal muttrc and the example mailbox in the
bugreport.
(SID)[EMAIL PROTECTED]:~$ mutt -f /tmp/muttbug/hebrew.mbox \
            -F /tmp/muttbug/muttrc -n -R
4. Voila.
--------------------

However I have just realized that it is depending on the terminal I
am using. I usually use mlterm, which shows the broken behavior[1],
however both xterm and rxvt-unicode-ml seem to be fine.

I am reassigning the bug to mlterm.

For reference I see the bug on stable and in a sid-chroot, the latter
therefore is a combination of etch's mlterm 2.9.3-5 using sid's
terminal definition (mlterm-common 2.9.4-3).

[1] with TERM=mlterm, TERM=xterm looks correct.
