That seems an encoding problem, where Ä is internally differently in xemacs
until the file is saved (lack of latin fonts unification). There have been a
lot of woody->sarge changes in both xemacs and ispell.el (now provided by
dictionaries-common package), so please check if that problem still holds.
I remember some possibly related problems appearing when LANG was an euro
variant (iso-8859-15), because the same char was considered internally
different if iso-8859-1 or iso-8859-15, so if you write as iso-8859-15 and
flyspell returns the corrected char as iso-8859-1, they are internally
considered as different, even being the same char. If that is the reason,
if you save that file, reopen it, and spellcheck it, Ä should be
spellchecked O.K.
I have tried to reproduce it, but everything seems to be working here,

flyspell-buffer only tags "änderung" and "dfsdfs" as mispelled. Please
confirm if that is the same at your site and we can close this bug
report.
