update-grub is call indirectly by the linux-image postinst, by way of the hooks in /etc/kernel-img.conf. These hooks are neither configured by nor maintained by the linux-image packages. /sbin/update-grub is marked as deprecated even in the 'grub' package (there's a NEWS.Debian file to warn users), so I suspect the answer is that users need to manually update their kernel-img.conf files.
