I observe the same problem as described by Sven and Francecso. Every now and then (maybe once a day) my mouse looses its "Emulate3Buttons" ability; pressing the wheel as third button still works. Restarting the X server solves the problem temporarily.
I don't think that (in my case) the problem is connected with any particular X application: when the problem occurs, pasting with left+right button fails with any application.
