I had a look at the provided headers, and saw nothing wrong with them. The cause must be elsewhere. Can you provide the output from '/usr/share/insserv/check-initd-order -g' and '/usr/share/insserv/check-initd-order -g -k', as well as the output from '/usr/share/insserv/make-testsuite'? It would make it possible to see the complete dependency graph for the start and stop sequences as well as all headers to be able to reproduce the problem.
