In /etc/nagiosgrapher/ngraph.d/standard/check_swap.ncfg, the ngraph template for "swap" has two lines that start with "definengraph", instead of "define ngraph". This causes the swap template to be completely ignored. Just adding in these two spaces fixes the problem.
