I've tried kphone 4.2. With kphone 4.2, I may set a "readwrite device"
in the audio dialog. "hw:1", which is alsa notation for the second sound
card (the first is hw:0) seems to be acceptable here. However, after
dialing, kphone dies with the following output:

CallAudio: Opening ALSA device for Input 
cannot set sample format (Invalid argument)

The called phone (my POTS home phone) rings anyway and I can take the
(silent) call. Probably the SIP session isn't terminated properly upon
kphone's unexpected death.
