I think falla crashed again in the same way sometime in the past hour. Is serial console output logged anywhere by Ganeti? Are there any other kernel messages prior to the first g_vfs_done() error? (The monit errors in your screenshot may have pushed other details off-screen; and (No log file within the VM would have it as the bug seems to affect disk I/O to all partitions; the monitor errors in your screenshot may have pushed other details off-screen, although ScrollLck-PageUp might work).
