but d/copyright still says

whereas lsdvd 0.17 actually came from
  https://qa.debian.org/watch/sf.php/lsdvd

Thus, the package is in violation of 
  12.5. Copyright information
    In addition, the copyright file must say where the upstream sources
    (if any) were obtained,
