I suspect that this line of enquiry is just going to make Debian's
exim config setup even more baroque and less useful to people who
actually use exim the way it was designed to be used.  Can't you just
document it, and close bugs as they come in with a note to the docs?  I
know it's not perfect, but I'm not completely convinced that running an
MTA behind a dial on demand setup is so normal an endeavor that we
should bend over backwards to support it out of the box.
