Hi, It seems that I found the same issue in a Xen environment (using linux-modules-2.6.26-2-xen-amd64). I experienced a crash twelve days ago and today. I raised the priority because of data loss: once the error appear, (root) partition is remounted read-only, and then, the system is rebooted (with difficulties: a daemon like mysqld fails to stop in a read only environment); on restart, automatic fsck fails, and some data are lost in the forced fsck process.
