I have a problem when I try to mount a CD image, regardless of whether
it's a physical CD in a drive, or an ISO image on the file system. I've

If it happens, the loop device is lost and cannot be detached anymore.
Swapping the physical CD also doesn't help. FWIW, the traceback above
happened when I tried to mount an ISO image on the file system, and
had NO CD in my CD drive.
