Package: initramfs-tools Version: 0.118 Followup-For: Bug #762984 i am hit by the same problem. as a temporary fix, i added the 'vgchange -ay' to /usr/share/initramfs-tools/scripts/local-top/lvm2 as adding it to /etc/initramfs-tools/scripts/local-top/vgscan somehow did not work (most likely due wrong order of execution, but i did not investigate further)
