This is actually a kernel issue I think, so reassigning accordingly. 2c3fc8d26dd0 "swiotlb-xen: pass dev_addr to swiotlb_tbl_unmap_single" was backported to the stable kernel but this commit was reverted in mainline via dbdd74763f1f.
