This one will not work with LO 4.0 due to API changes and removal of File → Templates submenu. See upstream bugs #60592 and #60660 <https://bugs.freedesktop.org/show_bug.cgi?id=60592> <https://bugs.freedesktop.org/show_bug.cgi?id=60660> BUT it should be listed on installed extensions list. It installs fine, it "just" don't work anymore. So if it is not listed there, then this is another issue.
